I think there was a similar incident at one of the big state hospitals up in KS around 12-15 years ago. There are also tons of other bad things (tm) going on in such institutions -- it's one of the big reasons there's such a push for privatization and group homes in the industry.
I used to work for a small group home with my ex, and she is back in the industry nowadays (after a seven or eight year stint in the daycare industry).
